IMPORTANT NOTE:

Please follow these instructions only if you have received a letter in the mail by your MAC (Medicare Administrative Contractor) provider. The letter will confirm that your case has been initiated within our database, and that will allow us to process your fingerprint submission. When a case is initiated in the database, we can verify if the applicant's case information has been entered correctly by CMS or if we need to edit information. Database information must match the data on the applicant's fingerprint cards or we cannot process your fingerprints.

FBI Channeler Information

Accurate Biometrics is the approved FBI channeler for CMS. Accurate Biometrics will process received fingerprint submissions to channel and initiate the fingerprint based background. Accurate Biometrics will receive a response to your request and provide it to CMS for review in a timely manner. Below are two options on how you may proceed with fingerprint collection.

By Mail Option

HARD CARDS

If our Live Scan is not an option for you, fingerprint cards (FD-258) should be sent via mail. (No photocopies or faxes are permitted/accepted)

Print out the CMS Fingerprint form packet:

Fingerprints should be placed on the FD-258 fingerprint card.

Hard copies of fingerprints should be captured by a local law enforcement agency or fingerprinting vendor. Individuals may not fingerprint themselves.

Contact your local law enforcement agency or fingerprint vendor to make arrangements to have your affidavit completed and hard copy ink fingerprints taken directly on each form.

Once fingerprints are collected, mail the following:

  • One original, complete Fingerprint Consent Form
  • One original, complete Fingerprint card: FD-258 (prefer not folded)
  • One original, complete Affidavit of Person Collecting Fingerprints
